Thank you for registering with Science Care and making your wish to donate to science known.
During the COVID-19 pandemic, we will not be mailing any material, including registration packets. For your convenience, below we are including links to an information brochure, a printable registration card, and the “Donation and Cremation Consent” authorization form, which should be completed and returned at your earliest convenience.
This form must be completed, initialed, printed, signed and dated by the registrant, and signed by two witnesses. Witness signatures do not need to be notarized. This should only take a few minutes to complete.

It is important to note that registration is not a guarantee of acceptance. Medical conditions change with regularity, and we have no way of knowing in advance if you would meet medical criteria for donation at the time of your passing.
Those who register with Science Care’s program do so as an
expression of intent, and not a contract. We encourage having a backup plan in place for your loved ones, even if it is just the name and phone number of a funeral home should there be a need for alternate arrangements.
Many people meet criteria for donation, including people with cancer, heart disease, arthritis, and diabetes. Please note that while we strive to facilitate every donation, a criteria match is contingent upon medical suitability at the time of passing or once a person is under end-of-life care for a terminal illness. Exclusions may include contagious diseases, extreme obesity, or family objection.
